Background The introduction of percutaneous mitral valve (MV) repair had an effect on clinical practice in comparison with surgical MV repair. Complete nationwide data are useful in examining how the introduction of a new technique influences clinical practice. Methods We analyzed procedural numbers, patient characteristics, and in-hospital outcomes for all percutaneous edge-to-edge and surgical MV reconstruction procedures performed in Germany between 2009 and 2015. Results 12,664 percutaneous edge-to-edge and 22,825 surgical MV reconstructions were recorded. Numbers increased steadily, albeit more rapidly in the percutaneous edge-to-edge group (108-4079 vs. 2923-3603 with surgical MV reconstruction). Patients with percutaneous edge-to-edge MV reconstruction were older (75.6 ± 8.8 vs 61.6 ± 13.4 years, P less then 0.001) and at higher operative risk (estimated logistic EuroSCORE 13.2% vs. 4.7%, P less then 0.001) compared to those undergoing surgery. However, in-hospital mortality did not differ (2.9% vs. 2.8%; P = 0.395). This was also true for the subset of 2103 patients at intermediate operative risk as defined by a logistic EuroSCORE ≥ 4% and ≤ 9%. Of note, complication rates (except acute kidney injury) were more favorable in patients undergoing percutaneous edge-to-edge reconstruction. Conclusions Percutaneous edge-to-edge MV reconstruction has markedly changed clinical practice of MR therapy in Germany. Annual overall procedural numbers more than doubled, with a massive increase in percutaneous edge-to-edge procedures. Our data demonstrate its use mainly in high-risk patients and prove the favorable safety profile of this novel technique, with low in-hospital mortality and complication rates.Introduction Atrial fibrillation (AF) is the most common type of arrhythmia. There is limited information on clinical pathologic changes of equine piroplasmosis in donkeys. This study aims to investigate the impacts of babesiosis on various hematological and biochemical properties in naturally infected donkeys with Theileria equi and Babesia caballi. Methods A total of 200 apparently healthy donkeys were randomly examined for the presence of equine piroplasmosis in blood samples. Of 200 samples, 11(5.5%) animals were positive for T. equi and 3(1.5%) of the samples were infected with B. caballi. 14 uninfected donkeys, were also selected as a control group. Results The results of the comparison showed that infected donkeys presented a significant elevation (P less then 0.05) of total leukocyte count, number of lymphocyte, monocyte, neutrophil and eosinophil, while showed a significant decreased in (P less then 0.05) hemoglobin concentration (Hb), packed cell volume (PCV), red blood cells (RBCs), mean corpuscular volume (MCV) and mean corpuscular hemoglobin concentration (MCHC). The infected donkeys had significantly higher levels of total protein and total bilirubin (P less then 0.05), whereas the amount of albumin was significantly lower in comparison to healthy animals (P less then 0.05). It is interesting to note that the amounts of urea, cholesterol, creatinine, triglyceride, LDL and HDL, CK, AST, LDH, GGT, and Malondialdehyde were significantly higher (P less then 0.05) in infected donkeys. Conclusions The current study describes laboratory findings in the affected donkeys which were good criteria for diagnosis and control of equine piroplasmosis in donkeys.Despite the common acceptance that the enthalpy of DNA duplex unfolding does not depend on temperature and is greater for the CG base pair held by three hydrogen bonds than for the AT base pair held by only two, direct calorimetric measurements have shown that the enthalpic and entropic contributions of both base pairs are temperature dependent and at all temperatures are greater for the AT than the CG pair. Each of these two processes is responsible for about half the Gibbs energy of duplex stabilization, but all the enthalpy, i.e., the total heat of melting, results from dissociation of the stacked base pairs. Both these processes tightly cooperate while the pairing of conjugate bases is critical for recognition of complementary strands, stacking of the flat apolar surfaces of the base pairs reinforces the DNA duplex formed.
