Notably, the therapeutic response of GLP-1R agonism in the setting of experimental influenza virus infection may have relevance for ongoing studies of GLP-1R agonism in people with T2D susceptible to viral lung injury.The structured life course modeling approach (SLCMA) is a theory-driven analytic method that empirically compares multiple prespecified life course hypotheses characterizing time-dependent exposure-outcome relationships to determine which theory best fits the observed data. In this study, we performed simulations and empirical analyses to evaluate the performance of the SLCMA when applied to genome-wide DNA methylation (DNAm). Using simulations, we compared five statistical inference tests used with SLCMA (n=700), assessing the family-wise error rate, statistical power, and confidence interval coverage to determine whether inference based on these tests was valid in the presence of substantial multiple testing and small effects, two hallmark challenges of inference from omics data. In the empirical analyses, we evaluated the time-dependent relationship of childhood abuse with genome-wide DNAm (n=703). In simulations, selective inference and max-|t|-test performed best both controlled family-wise error rate and yielded moderate statistical power. Empirical analyses using SLCMA revealed time-dependent effects of childhood abuse on DNAm. Our findings show that SLCMA, applied and interpreted appropriately, can be used in high-throughput settings to examine time-dependent effects underlying exposure-outcome relationships over the life course. We provide recommendations for applying the SLCMA in omics settings and encourage researchers to move beyond analyses of exposed versus unexposed.The beta-interferons are widely prescribed platform therapies for patients with multiple sclerosis (MS). We accessed a cohort of patients with relapsing onset MS from British Columbia, Canada (1995-2013) to examine the potential survival advantage associated with beta-interferon exposure using a marginal structural model. Accounting for potential treatment-confounder feedback between comorbidity, MS disease progression and beta-interferon exposure, we found an association between beta-interferon exposure of at least 6 contiguous months and improved survival (hazard ratio (HR) = 0.63, 95% confidence interval 0.47-0.86). We also assessed potential effect modifications by sex, baseline age or baseline disease duration, and found these factors to be important effect modifiers. Once established, this model was then used to measure the effectiveness of a siderophore receptor and porin (SRPĀ®) APEC vaccine. Initially, five pilot studies were performed on egg-laying chickens to compare Escherichia coli ( E. coli ) serotype, challenge route, and dose of inoculum that resulted in pathology characteristic of peritonitis observed in commercial layer facilities such as widespread organ infection, atrophy, discoloration, corrugation of yolk sacs and the presence of caseous exudate. Isolates of serotype O1, O2 and O78 were tested by intravenous (IV), intravaginal (IVAG), intratracheal (IT) and intraperitoneal (IP) routes and were compared at varying levels of challenge inoculum. Daily observations of mortality and morbidity were made and, at necropsy, gross lesion scores were collected and bacterial colonization of internal organs determined. Outcomes variedne effectiveness was dramatic and was shown to completely protect against mortality, and substantially against tissue colonization and pathology typical of APEC infections. The vaccine elicited a rapid onset of immunity with both narrow and broad vaccination intervals and in both young and mature chickens. Additionally, the vaccine was demonstrated to sustain robust effectiveness against mortality over a three-month duration. The SRP APEC vaccine should provide effective protection of young and mature chickens from E. E7766 coli under broadly flexible conditions of use in commercial operations.

    Delirium is underdiagnosed in clinical practice, and is not routinely coded for billing. Manual chart review can be used to identify the occurrence of delirium, however, it is labor-intensive and impractical for large-scale studies. Natural language processing (NLP) has the capability to process raw text in electronic health records (EHRs) and determine the meaning of the information. We developed and validated NLP algorithms to automatically identify the occurrence of delirium from EHRs.

    This study used a randomly selected cohort from the population-based Mayo Clinic Biobank (n=300, age>=65). We adopted the standardized evidence-based framework confusion assessment method (CAM) to develop and evaluate NLP algorithms to identify the occurrence of delirium using clinical notes in EHRs. Two NLP algorithms were developed based on CAM criteria; one based on the original CAM (NLP-CAM; delirium vs. no delirium) and another based on our modified CAM (NLP-mCAM; definite, possible, and no delirium). As the usage of DiNP worldwide increases, studies evaluating its potential reproductive toxicity are becoming more prevalent in the literature. In studies on female animals, the researchers found that the exposure to DiNP appears to induce negative effects on ovarian function and fertility in animal models. Whether or not DiNP has direct effects on the uterus is still controversial, and the effects on human reproduction require much more research.
